Sacramental Records

    Marriage

    If a parish, give the name of parish and address below. If location is not a religious one, the name of the location or description (i.e. Hilton Inn or Groom's Home or Local Beach)
    If either or both spouse's plan to change their last names, give the name they had at birth in both spaces.
    Whether living or deceased, present or not. If the parent is estranged and the spouse doesn't want them mentioned, don't press the issue.
    Each spouse should have one witness, traditionally called "Best Man" and "Maid of Honor". However, if only one witness was present, that's fine.
    Make note of whether one or both partners were baptized. At least one should be for us to perform the ceremony. If neither are baptized but they have asked for us to perform the ceremony, check with your bishop for permission.
